Similarly, bleeding from the adrenal or spleen can be efficaciously man- aged by adrenalectomy or splenectomy, respectively.

A suction drain is only necessary in cases of pancreatic injury, to minimize the risk of serious complications Montague Extensive pancreatic injuries, typically to the tail of the pancreas during left-sided adrenal or renal surgery, necessitate a distal pancreat- ectomy. This can be carried out laparoscopically with a stapling device; however, if an open duct is identified, it must be closed with a figure-of-eight suture.

Intraoperative consultation from a general sur- geon should be sought for any doubtful cases. However, if ventilation is impaired during the case, venting the pressure in the pleural cavity with an angio- catheter may be necessary. Laparoscopic repair of most pleural injuries is advocated to minimize postoperative patient morbidity.
Diaphragmatic injuries are more con- siderable, and primary full-thickness closure of the diaphragm using figure-of-eight non-absorbable sutures, placed under low insufflation pressures, achieves durable repair. An alternative approach is placement of a 6-Fr catheter with extra cut side-holes into the pleural space via the sixth intercostal space to aspirate residual air.
It is not uncommon to observe small amounts of residual pneumothorax postoperatively, which can be managed expectantly since carbon dioxide will be rapidly absorbed. However, if patients are symptomatic with respiratory insufficiency, a small chest tube may be placed for relief. Continued technological advance- ments and improved surgical skills have prompted laparoscopists to undertake complex reconstructive procedures.

Goldman is fellowship trained and board certified in the subspecialty of female pelvic medicine and reconstructive surgery. He has a joint appointment in the departments of urology and obstetrics and gynecology, is actively involved in the training of fellows and residents and is the past Fellowship director for Female Pelvic Medicine and Reconstructive Surgery in the Department of Urology at the Cleveland Clinic.
His interests are in the medical and surgical treatment of urinary incontinence and other types of voiding dysfunction, neuromodulation, prolapse repair, complex redo female urologic surgery, neurourology and fistula repair. He has authored over articles and book chapters, is the editor of three textbooks, has been a visiting professor in the United States and abroad and given numerous invited lectures.
The end result is an exciting survey of the practical applications of the latest techniques.
Joining Professor Hohenfellner as Editor is Professor John Fitzpatrick, one of the most prominent figures on the world urology scene and the editor of leading urology journal, BJU International. All chapters include a list of key references explaining the practical application of the latest techniques in urologic surgery - essential reading for urologists in both practice and training.
